I suggest a letter to your boss with the obligatory "thank you for the opportunity, I have very much appreciated being part of your team, but have decided to move on to new opportunities" in a very positive sort of voice and of course the last day that you will be working. Two weeks notice, of course, unless you've promised to give more notice than that/don't have an obligation to give that much notice.
